Client Informed Consent
By submitting a consultation request, the client confirms that the information provided is accurate to the best of their knowledge and voluntarily consents to professional review by an appropriately assigned psychologist.
The client understands that online, telephone and written consultation services have limitations and may not be suitable for emergencies, complex diagnostic procedures or situations requiring physical examination.
The client consents to the use of submitted information for consultation delivery, appointment coordination, professional assignment, service administration and secure communication.
The client understands that confidentiality may be limited where disclosure is required by law, where there is a serious and immediate risk of harm, or where safeguarding obligations apply.
The client may ask questions about the process before proceeding and may withdraw from future services, subject to the applicable refund and cancellation policy.
